A Simple Guide to Medicare in California

Medicare doesn't have to be complicated. This guide is written for California residents who are new to Medicare — or who just want a clear, honest overview without the insurance industry jargon. Let's start from the beginning.

What Is Medicare?

Medicare is the federal health insurance program primarily for people 65 and older, though it also covers some people with disabilities or certain medical conditions at a younger age. It's funded through Medicare taxes you've paid throughout your working life, and it's available regardless of your income or health status.

The Four Parts of Medicare

Part A — Hospital Insurance

Part A covers inpatient hospital care, skilled nursing facility care (after a qualifying hospital stay), hospice care, and some home health services. For most people who worked in the U.S. for 10 or more years, Part A is free — no monthly premium.

Part B — Medical Insurance

Part B covers outpatient medical services: doctor visits, preventive care, lab tests, X-rays, outpatient surgery, durable medical equipment, and mental health services. Part B has a monthly premium — $185/month in 2025 for most people — and a $257 annual deductible. After the deductible, Medicare covers 80% of covered services; you typically pay 20%.

Part C — Medicare Advantage

Medicare Advantage is an alternative way to get your Medicare benefits through a private insurance company instead of directly through the federal government. Medicare Advantage plans bundle Part A and Part B coverage (and usually Part D drug coverage) into one plan. Many plans offer $0 premiums and extras like dental and vision. California has excellent Medicare Advantage plan options, especially in the San Diego area.

Part D — Prescription Drug Coverage

Part D is Medicare's prescription drug benefit. It's offered through private insurers and covers a wide range of brand-name and generic medications. Premiums, deductibles, and copays vary by plan. Starting in 2025, there's a $2,000 annual cap on out-of-pocket drug costs — a significant new protection.

How Do I Sign Up for Medicare in California?

You typically sign up for Medicare through the Social Security Administration, either online at SSA.gov, by calling 1-800-772-1213, or by visiting a local Social Security office. In California, you can also get enrollment help through:

  • HICAP (Health Insurance Counseling and Advocacy Program) — free state counseling
  • A local independent Medicare broker like Ashley Watson — free, personalized help

When Should I Sign Up?

Your Initial Enrollment Period is a 7-month window: 3 months before your 65th birthday month, your birthday month itself, and 3 months after. Enroll early in this window for the earliest possible coverage start date.

If you're still working and covered by an employer plan at 65, you may qualify to delay Medicare Part B without penalty — but the rules depend on your employer's size. (See our article on Medicare vs. Employer Coverage for details.)

What Does Medicare Cost in California?

Here's a simple cost breakdown for 2025:

Part Monthly Premium Deductible
Part A $0 for most people $1,676 per benefit period (hospital)
Part B $185/month (standard) $257/year
Part D Varies by plan (~$0–$100+) Up to $590/year
Medicare Advantage (Part C) Often $0 (includes A, B, D) Varies by plan
Medigap Supplement $80–$200+/month (adds to Part B) Typically very low

Higher-income beneficiaries may pay more for Part B and Part D through IRMAA (Income-Related Monthly Adjustment Amount) surcharges.

Original Medicare vs. Medicare Advantage: Which Is Better?

This is the central choice most people face. In short:

  • Original Medicare (Parts A + B) lets you see any doctor in the country who accepts Medicare, with no network restrictions. You'd pair it with a Part D plan and possibly a Medigap plan for comprehensive coverage.
  • Medicare Advantage (Part C) bundles everything into one plan, often with $0 premiums and extras like dental and vision. You use a provider network and may need referrals for specialists.

Neither is universally better. The right choice depends on your health, your doctors, how much you travel, and your budget. An independent broker can help you compare honestly.

California-Specific Medicare Protections to Know

California offers a few Medicare-related protections beyond what federal law requires:

  • The Birthday Rule: Each year during a 60-day window around your birthday, you can switch to an equal or lesser Medigap plan with guaranteed acceptance — no health questions. This gives California residents more flexibility to change Medigap plans than most other states allow.
  • Medi-Cal integration: California's Medicaid program (Medi-Cal) has relatively generous eligibility thresholds, and many California seniors qualify for assistance they don't know about.

Frequently Asked Questions

Does Medicare cover everything?

No. Original Medicare covers most medically necessary care but has significant gaps — it pays 80% of most Part B costs (you pay 20%), has hospital deductibles, and generally doesn't cover routine dental, vision, or hearing care. Medigap plans and Medicare Advantage plans help fill these gaps.

Is Medicare free?

Part A is usually free. Part B has a monthly premium ($185 in 2025). Medicare Advantage plans often have $0 additional premiums on top of Part B. Medigap plans add a monthly premium. So "free" depends on the choices you make.

Do I have to choose a plan, or does Medicare just happen automatically?

You need to actively enroll. If you receive Social Security benefits before 65, you may be automatically enrolled in Parts A and B, but you still need to actively choose a Medicare Advantage or Part D plan if you want that coverage.
