If you're a San Diego Unified retiree on a UnitedHealthcare Medicare Advantage plan, your coverage is changing to Humana on January 1, 2027. My phone has been ringing nonstop about this, so I wrote this guide to answer the questions I'm hearing most.

What's happening

San Diego Unified gets its retiree medical plans through the CA Schools VEBA trust. The trust is replacing all UnitedHealthcare Medicare Advantage plans, both the PPO and the HMO, with two new Humana Medicare Advantage PPO plans. According to the district, the reason was an unfavorable renewal from UnitedHealthcare that would have significantly raised monthly premiums.

Who this affects:

  • Affected: Retirees and spouses on the district's UHC Medicare Advantage PPO or HMO.
  • Not affected: Retirees on Kaiser Senior Advantage, which continues (and adds a hearing aid benefit for 2027).
  • Not affected: Family members under 65 who aren't on Medicare. They stay on their current UHC plan.

You don't have to fill out any paperwork to move to Humana. It happens automatically. But "automatic" doesn't mean "no impact," especially if you have doctors you want to keep.

Which Humana plan will you be moved to?

If you're on this todayYou'll move to this on Jan 1, 2027
UHC Medicare Advantage PPOCalVEBA RetireeFirst Humana $0 PPO
UHC Medicare Advantage HMOCalVEBA RetireeFirst Humana $15 PPO

The district says the new Humana plans are designed to closely mirror your current UHC benefits. The two Humana plans have different copays and costs, so it's worth reviewing the official plan summaries before January.

You'll get two ID cards: one for medical and one for prescriptions. Both plans include Part D drug coverage, so don't sign up for a separate drug plan. Doing that would disenroll you from the district plan.

For exact benefits and costs, see the plan summaries on the district's retiree benefits page or call RetireeFirst (contact info below). The plan's Evidence of Coverage has the full details.

Can you keep your doctors?

It depends on whether your doctor's office will accept the Humana plan. Both new plans are "passive" PPOs, which means you pay the same copay in or out of network, as long as the provider participates in Medicare and agrees to accept the plan. That last part is the catch. A provider can decline.

Important: Sharp Rees-Stealy does not accept the Humana PPO

If your doctors are with Sharp Rees-Stealy Medical Group, this change hits you hardest. Sharp Rees-Stealy's own insurance page says it does not accept Medicare Advantage PPO plans. It does accept UHC Medicare Advantage HMO plans, which is why so many district retirees chose the UHC HMO in the first place.

So if you're on the UHC HMO today with Sharp Rees-Stealy doctors, you're being moved to a plan your medical group won't take. Don't wait until January to find out at the front desk.

What to do now

  1. Make a list of every doctor you see, including specialists, plus your hospital and pharmacy.
  2. Call each office and ask: "Will you accept the CalVEBA RetireeFirst Humana Group Medicare Advantage PPO in 2027?"
  3. Check your prescriptions against the Humana drug list.
  4. If a key doctor says no, look at your options in the next section before the district's deadline.

Your options

According to the district's open enrollment letter, you have three paths.

OptionGood fit ifWatch out for
Stay on the Humana PPO (no action needed)Your doctors accept it, or you're open to new doctorsConfirm every provider before January
Switch to Kaiser Senior Advantage (district plan)You're willing to move your care to KaiserMust be done during district open enrollment, by Nov 6
Leave the district plan for individual Medicare coverageKeeping a specific medical group, like Sharp Rees-Stealy, is your top priorityYou can't come back (see below)

Think hard before leaving the district plan

This is the most important thing in this article. Per the district, if you voluntarily cancel your district medical coverage, it cannot be reinstated. That's a permanent decision. Before you leave, find out:

  • Your subsidy. Some retirees get a district Retiree Medical Fund subsidy based on their bargaining unit. Ask whether you'd lose it.
  • Your costs. Group retiree plans often have lower out-of-pocket limits than individual Medicare Advantage plans. Compare your total costs carefully before switching.
  • Your spouse. A change for you may affect a covered spouse or dependent.
  • Medigap underwriting. If you're considering Original Medicare with a Medicare Supplement, you may have to answer health questions. Whether you qualify without health questions depends on your situation. Call or text me at 619-947-2325 and I'll help you find out.

For some people, keeping a trusted doctor is worth it. For others, the group plan's low costs win. There's no one right answer, which is why it's worth a side-by-side review.
